| General Site Plan Form | This form supplies information about the project, such as address, owner's name, ward, parcel number, and telephone number. |
| Site Plans Check List | Description of all potential information about the project and summary. |
| ACAD Checklist | Description of all information necessary to complete an ACAD drawing using BWSC's AutoCad Standards. |
| Back Flow Preventer (BFP) | Required to gain approval for the installation of backflow preventor devices. Also includes regulations and guidelines. |
| Dewatering Permit Form | Required to gain approval for any dewatering into the Commission’s systems |
| Termination Verification Form | Required to gain approval from the City of Boston Inspectional Services Department for demolition of structures within the Cities jurisdiction. |
| Easement Request Form | Required when the applicant wishes to grant the Boston Water and Sewer Commission an easement (a right in private land or a private way). |
